WHAT’S A HEALTH COACH?
Most people have never had a health coach. That’s because most people have never had healthcare that shows up between checkups. This page is for anyone wondering what changes when a coach does.
A Shape Health Coach is a trained Community Health Worker who meets with you every week to help you build the daily habits that protect your health. Not your doctor. Not a nutritionist. Your day-to-day partner who knows your name, your kitchen, and what’s getting in your way.

WHAT MAKES A SHAPE COACH
Anyone can call themselves a wellness coach. Our coaches are something different. Every member of our coaching team carries the same four credentials.
State-certified through NM Health Care Authority. The same credential your hospital and FQHC trust.
Behavior change, motivational interviewing, nutrition basics, care coordination, cultural humility, and Medicaid navigation.
Every coach has a weekly check-in with our clinical lead. Your care plan is reviewed and updated with a clinician in the loop.
Your information is handled with the same standards as your doctor’s office. We are contracted with NM Medicaid and audited regularly.

No. Your coach works alongside your doctor. They send notes back to your primary care after every visit and help you stay on top of appointments, prescriptions, and follow-ups.
Yes. The relationship matters. If your coach isn’t the right match, we’ll pair you with someone who is. No awkward conversation needed. Just message our team.
No. That’s your doctor’s job. Coaches help you live with what you’ve been diagnosed with and prevent what hasn’t been yet. They flag concerns to our clinical team when needed.
Most members don’t, at first. That’s the point. Your first visit is mostly your coach listening. The “what to work on” emerges from the conversation. There’s no test to pass.
